When This Medicine Should Not Be Used

You should not use this medicine if you have had an allergic reaction to loratadine or pseudoephedrine. If you have glaucoma, problems urinating, severe high blood pressure, or heart disease or if you have taken a type of medicine called an MAO inhibitor (such as Marplan®, Parnate®, or Nardil®) in the past 14 days, then you should not take this medicine. Do not give any over-the-counter (OTC) cough and cold medicine to a baby or child under 2 years old. Using these medicines in very young children might cause serious or possibly life-threatening side effects.

How to Use This Medicine

12 Hour Tablet, Long Acting Tablet, 24 Hour Tablet

Your doctor will tell you how much to take and how often. You should not use more of this medicine than your doctor ordered.
Claritin-D 24 Hour® extended-release tablets should only be taken once a day.
Take the tablet with a full glass of water.
Swallow the tablet whole. Do not break, crush, or chew.
It is best to take this medicine on an empty stomach.
Drink plenty of liquids while taking this medicine.

If a dose is missed:

Take your medicine as soon as possible unless it is almost time for your next dose. If you are taking Claritin-D 24 Hour®, you should only take one dose a day.
Skip the missed dose if it is almost time for your next regular dose.
You should not use two doses at the same time.

How to Store and Dispose of This Medicine

Keep the medicine at room temperature in a closed container, away from heat, moisture, and direct light.
Keep all medicine out of the reach of children.

Avoid drinking alcohol while taking this medicine.

If a dose is missed:

Tell your doctor if you have severe heart disease, very high blood pressure, or trouble urinating. Make sure your doctor knows about any reaction you have had to other drugs, especially if it involved trouble sleeping, dizziness, weakness, shaking, or changes in your heart rhythm.

How to Use This Medicine

Your doctor will tell you how much of this medicine to use and how often. Do not use more medicine or use it more often than your doctor tells you to.

Angioedema refers to swelling that occurs in the tissue just below the surface of the skin, most often around the lips and eyes. It generally results from an allergic reaction to either a food or medication. More rarely, it may be a sign of an underlying condition such as leukemia or Hodgkin's disease. In an allergic reaction, the body produces histamine, which causes blood vessels to swell. There are two basic types of angioedema:

Hereditary angioedema (HAE), which is genetic and tends to recur
Acquired angioedema (AAE)

Angioedema can take anywhere from minutes to hours to develop. Interestingly, angioedema may affect an area on one side of the body but not on the other. In most cases, angioedema is mild. Severe angioedema can cause the throat or tongue to swell, cutting off the airway, and it can be life-threatening.

Signs and Symptoms

Common symptoms of angioedema include:

Sudden appearance of red welts, especially near the eyes and lips, but also on the hands, feet, and the inside of the throat
Burning, painful, swollen areas; sometimes itchy
Hoarseness, tight or swollen throat, breathing trouble
Discolored patches or rash on the hands, feet, face, or genitals
Vomiting, abdominal pain, diarrhea, and reduced appetite
In a form called angioedema-eosinophilia syndrome, hives, itching, fever, muscle pain, decreased urine, weight gain, and high white blood cell count occur.

What Causes It?

Sometimes the cause is difficult to identify. An angioedema reaction (AAE in particular) may be caused by allergies to foods, dyes, or pollen, or in reaction to certain medications. Foods that commonly cause allergies include shellfish, dairy, and nuts. Drugs that often spark allergic reactions include nonsteroidal anti-inflammatory drugs (NSAIDs, such as ibuprofen), blood pressure medication, aspirin, and antibiotics. Conditions such as leukemia, Hodgkin's disease, and connective tissue disorders (such as lupus) may also trigger angioedema.

Who's Most At Risk?

These factors increase the risk for angioedema:

Having hives or angioedema before
Having had a body-wide allergic reaction in the past
Experiencing injury, sudden temperature change, stress, or anxiety
Exercising intensely
Undergoing a dental procedure
Having ovarian cysts

What to Expect at Your Provider's Office

Your health care provider will perform a physical exam and ask about your symptoms. Be sure to tell him or her about all medications (prescription and over-the-counter), herbs, and supplements you are taking. Blood and urine tests may help pinpoint the cause of the angioedema.

Treatment Options

Prevention

You should eliminate any known or suspected triggers. Allergy testing with a trained specialist may help identify inciting agents. If you are prone to angioedema, you should wear a Medic Alert bracelet that notes this condition.

Treatment Plan

The first priority is to ensure that the airway is open and that breathing is not impaired. The next steps include identifying and removing the trigger as well as relieving other symptoms. Infrequent attacks can be managed as they arise. Frequent attacks may require ongoing treatment, perhaps with an allergist, dermatologist, or other specialist to try to avoid recurrences.

Drug Therapies

Several medicines may help prevent or relieve attacks. For mild cases, you can use over-the-counter antihistamines, such as Benadryl, Alavert, or Claritin. Note that Benadryl often causes drowsiness.

You may use prescription antihistamines. For severe cases, your doctor may prescribe corticosteroids to reduce swelling and itching, or you may need a shot of epinephrine. Mild attacks tend to clear up within 4 days with or without medication.

Complementary and Alternative Therapies

In a severe attack, standard emergency medical care should be administered immediately to open airways and stabilize the condition. No new substances, including herbs or supplements, should be introduced during an acute attack.

Long-term nutritional and herbal support used in between attacks may help reduce or prevent angioedema. In addition, herbs and supplements may help alleviate mild symptoms, particularly for chronic and recurring forms. Homeopathic remedies may help alleviate mild symptoms and reduce the frequency and severity of episodes. It is important to tell your doctor about all medications, herbs, and supplements you are taking.

Nutrition and Supplements

Certain foods and food additives may trigger angioedema in people who are susceptible. You should eliminate any foods or food additives that trigger symptoms. The following are the most common food triggers:

Seafood
Nuts
Legumes
Eggs
Chocolate
Milk
Berries

Certain individuals may have a reaction in response to:

Citrus fruits
Sulfites -- used as an antioxidant or freshening agent (preservative) in many foods and beverages
Yellow dye No. 5 (also called tartrazine) -- those who are allergic to aspirin or other NSAIDs are more susceptible to having an allergy to yellow dye No. 5

Health care providers can help identify food triggers by:

Collecting detailed information about your diet
Performing skin tests for sensitivity to certain substances
Cautiously testing suspected triggers
Monitoring symptoms as foods are eliminated from your diet, then slowly re-introduced one at a time

If you have gastrointestinal symptoms (abdominal pain, vomiting, diarrhea, or reduced appetite), this may be an indication that you absorb antigens (food triggers) more easily than others through the stomach and intestines. If so, you may benefit from a diet that eliminates common dietary antigens (as listed above) even if you do not have a specific, identifiable food allergy.

These supplements may also help treat symptoms:

Vitamin C (2 - 4 g per day) may help lower histamine levels. Reduce dose if diarrhea develops.
Vitamin B12 (1,000 mcg per week by injection) may help reduce the frequency of chronic attacks.
Quercetin (a naturally occurring flavonoid) may reduce the likelihood of an allergic reaction. The recommended amount is generally 200 - 400 mg three times a day before meals. If you are sensitive to citrus or take calcium-channel blockers to treat high blood pressure, you should avoid citrus-based forms of flavonoids. Quercetin is best absorbed in its water-soluble form, often referred to as hesperidin methyl chalcone (HMC). Few manufacturers make this form, so it can be harder to find. HMC Plus, made by Thorne Research, is one high-quality brand. Others are avialable. Talk to your doctor about which form of quercetin is best for you.
Bromelain is sometimes used to help reduce inflammation;. You can take it with curcumin (Curcuma longa), the yellow pigment of turmeric, which may enhance its effects. Avoid bromelain if you are allergic to citrus.

Herbs

Herbs are generally a safe way to strengthen and tone the body's systems. As with any therapy, you should work with your health care provider to get your problem diagnosed before starting any treatment. You may use herbs as dried extracts (capsules, powders, teas), glycerites (glycerine extracts), or tinctures (alcohol extracts). Unless otherwise indicated, you should make teas with 1 tsp. herb per cup of hot water. Steep covered 5 - 10 minutes for leaf or flowers, and 10 - 20 minutes for roots. Drink 2 - 4 cups per day. You may use tinctures alone or in combination as noted.

Green tea may have antihistamine properties. Drink three cups per day. If caffeine is a problem for you, drink decaffeinated coffee.
Devil's claw (Harpagophytum procumbens, 2 - 2.5 g per day) may reduce inflammation and skin lesions. Do not take devil's claw if you take blood pressure medication or blood-thinning medication, or if you have diabetes.
Goldenseal (Hydrastis canadensis, 250 - 500 mg three times per day) has been used for gastrointestinal symptoms such as stomach pain, diarrhea, and reduced appetite that may accompany a severe allergic reaction. Some health care providers also recommend goldenseal for those with food allergies to prevent or lessen reactions
Licorice root (Glycyrrhiza glabra, 200 - 400 mg 3 times per day) has been used traditionally to lessen inflammation in the case of an allergic reaction. It may also normalize immune function. Do not take licorice if you have heart disease, high blood pressure, or edema.

Homeopathy

Before prescribing a remedy, homeopaths take into account a person's constitutional type -- your physical, emotional, and psychological makeup. An experienced homeopath assesses all of these factors when determining the most appropriate treatment for each individual.

Apis is traditionally used for hives and angioedema and may be useful to prevent or treat chronic, recurrent cases.

Acupuncture

Some doctors report that acupuncture may help re-establish immune balance and lessen the frequency or severity of allergic reactions such as angioedema.

Prognosis and Possible Complications

If angioedema affects the throat, the airway passage could be blocked, thereby creating a life-threatening situation. It is also possible that the angioedema may develop into anaphylaxis, which will require emergency medical care to maintain breathing, blood pressure, and heart function and to reverse the reaction.

Following Up

After an attack, it's important to identify and avoid any triggers and to treat any underlying condition.

Using this medicine while you are pregnant can harm your unborn baby. Use an effective form of birth control to keep from getting pregnant. If you think you have become pregnant while using the medicine, tell your doctor right away.
Make sure your doctor knows if you are breastfeeding, or if you have lung disease, kidney disease, liver disease, heart disease, low blood pressure, thyroid disorder, diarrhea, or low potassium or magnesium in your blood.
You may need to have regular blood tests or eye examinations while you are using this medicine. Make sure to tell your doctor about all other symptoms that you have while using this medicine, including eye pain or any changes in vision.
Make sure any doctor or dentist who treats you knows that you are using this medicine. You may need to stop using this medicine several days before having surgery or medical tests.

Possible Side Effects While Using This Medicine

Call your doctor right away if you notice any of these side effects:

Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing.
Blistering, peeling, red skin rash.
Change in how much or how often you urinate.
Chest pain or discomfort.
Confusion and other mental changes.
Dark-colored urine or pale stools.
Eye pain or changes in vision.
Fast, slow, or irregular heartbeat.
Fever, chills, cough, sore throat, and body aches.
Lightheadedness or fainting.
Pain, swelling, redness, or rash at the site of injection.
Severe diarrhea, nausea, vomiting, or stomach pain.
Shortness of breath, cold sweat, and bluish-colored skin.
Trouble breathing, tightness in your chest.
Unusual bleeding, bruising, or weakness.
Yellowing of your skin or the whites of your eyes.

If you notice these less serious side effects, talk with your doctor:

Mild nausea, vomiting, or diarrhea.

Amiodarone (a-MEE-oh-da-rone)

Treats life-threatening heart rhythm problems (arrhythmias) in patients who have already taken other antiarrhythmic medicines.

When This Medicine Should Not Be Used

You should not use this medicine if you have had an allergic reaction to amiodarone or iodine, or if you are pregnant. You should not use this medicine if you have certain heart problems such as heart block or severe sinus-node dysfunction. Do not use this medicine if you have had fainting spells caused by very slow heartbeat.

How to Use This Medicine

Tablet

Your doctor will tell you how much of this medicine to use and how often. Your dose may need to be changed several times in order to find out what works best for you. Do not use more medicine or use it more often than your doctor tells you to.
You will receive your first dose of this medicine in a hospital. Your caregivers will watch you closely after you take this medicine to make sure you do not have any serious side effects.
This medicine should come with a Medication Guide. Read and follow these instructions carefully. Ask your doctor or pharmacist if you have any questions. Ask your pharmacist for the Medication Guide if you do not have one. Your doctor might ask you to sign some forms to show that you understand this information.
It may take one or two weeks before your body responds to this medicine. Take this medicine exactly as your doctor ordered, even if you feel fine.
You may take this medicine with or without food, but take it the same way each time.
You may need to carry identification showing that you are taking amiodarone. Ask your doctor about this.

If a dose is missed:

If you miss a dose or forget to use your medicine, use it as soon as you can. If it is almost time for your next dose, wait until then to use the medicine and skip the missed dose. Do not use extra medicine to make up for a missed dose.

How to Store and Dispose of This Medicine

Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light.
Ask your pharmacist, doctor, or health caregiver about the best way to dispose of any leftover medicine after you have finished your treatment. You will also need to throw away old medicine after the expiration date has passed.
Keep all medicine away from children and never share your medicine with anyone.

Drugs and Foods to Avoid

Ask your doctor or pharmacist before using any other medicine, including over-the-counter medicines, vitamins, and herbal products.

Make sure your doctor knows if you are using medicine to treat HIV or AIDS (such as indinavir, Crixivan®), medicine for allergy symptoms (such as loratadine, Claritin®, Alavert®), stomach medicine (such as cimetidine, Tagamet®), medicine to treat depression (such as trazodone, Desyrel®), or medicine that weakens your immune system (such as cyclosporine, methotrexate, Gengraf®, Neoral®, Sandimmune®, Folex®, Rheumatrex®).

Make sure your doctor knows if you are breastfeeding, or if you have heart disease (such as congestive heart failure), breathing problems or lung disease, liver disease, high or low blood pressure, thyroid disorder, or low potassium or magnesium in your blood.
Do not stop using this medicine suddenly without asking your doctor. You may need to slowly decrease your dose before stopping it completely.
Make sure any doctor or dentist who treats you knows that you are using this medicine. You may need to stop using this medicine several days before having surgery or medical tests.
This medicine may make your skin more sensitive to sunlight. Use a sunscreen when you are outdoors. Avoid sunlamps and tanning beds.After taking amiodarone for several months, your skin may look blue-gray in color, especially on areas exposed to sunlight.
Your doctor will need to check your progress at regular visits while you are using this medicine. Be sure to keep all appointments. You may also need to have regular blood tests or eye examinations while you are using this medicine.
Make sure any doctor or dentist who treats you knows that you are using this medicine. This medicine may affect the results of certain medical tests.

Possible Side Effects While Using This Medicine

Call your doctor right away if you notice any of these side effects:

Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing.
Blistering, peeling, red skin rash.
Blurred vision, seeing halos, feeling that your eyes are more sensitive to light.
Continued or worsened irregular heartbeat.
Cough with blood or chest pain.
Dark-colored urine or pale stools.
Fast, slow, or pounding heartbeat.
Lightheadedness, dizziness, or fainting.
Nausea, vomiting, loss of appetite, pain in your upper stomach.
Numbness, tingling, or burning pain in your hands, arms, legs, or feet.
Severe headache, pain behind your eyes, or problems with balance, walking, or speech.
Shallow or labored breathing.
Shortness of breath, cold sweat, and bluish-colored skin.
Swelling in your hands, ankles, or feet.
Twitching or muscle movements you cannot control (often in your eyes, jaw, neck or upper body).
Unusual bleeding or bruising.
Unusual tiredness or weakness.
Yellowing of your skin or the whites of your eyes.

If you notice these less serious side effects, talk with your doctor:

Dry eyes.
Loss of interest in sex.
Mild nausea, vomiting, stomach pain, constipation, or loss of appetite.
Trouble sleeping.
Warmth or redness in your face, neck, or chest.

Information

For common minor symptoms and illnesses, over-the-counter medicines are available. Always follow the manufacturer's directions and warnings when using these products. If you have any questions, consult with your doctor before starting a new over-the-counter drug. All medications lose their effectiveness (expire) over time and eventually should be replaced. Check the expiration date before using any product.

Remember that storing medicines improperly shortens their "shelf life." Store medicines in a cool, dry area. The heat from kitchens and the moisture from bathrooms decrease the effectiveness of a medication. Store all medications out of reach of children. See: storing medicine safely

Do NOT give aspirin to children under 12. Consult your health care provider before giving aspirin, ibuprofen, or similar medicines to children. Pepto-Bismol contains aspirin-like salicylates, so talk with your doctor before using it in children.

Avoid combination cough and cold preparations in children under age 6 because they have sometimes caused over-sedation and death. Above all, be sure that the medications you use do not contain the same ingredients, which could lead to overdoses.

Women should consult their doctor before taking over-the-counter medicines while pregnant or breastfeeding.

The following list includes some examples of over-the-counter medicines for common symptoms. This list is not comprehensive, nor does it imply an endorsement of some brands over others. Many of these have less expensive “store brand” equivalents. Always look at the ingredients. There are many “multisymptom” combination versions -- you should select products with the fewest ingredients needed to address your actual symptoms.

That Little Post Nasal Drip May Be Telling You Something

Seasonal allergies come with their fair share of fun and one of those good times is post nasal drip. Creating mucus is one of your body's lines of defense in attempts to flush allergens out of your nose, but if you're stuffed up and can't blow the mucus out, you'll experience post nasal drip. All that dreaded mucus builds up in your nose, leading to the sensation that mucus is dripping down the back of your nose and into your throat. It is not only a little bit gross, but it can also cause a chronic sore throat. The fun doesn't end there because the mucus can also drip into your lungs causing wheezing, difficulty breathing, and the need to clear your throat often.

The best means of warding off post nasal drip is to get to the root of your seasonal allergies and avoid those allergens. If you are allergic to the entire great outdoors (I exaggerate a bit, but sometimes it feels that way) you might want to try allergy medication. There are many over-the-counter antihistamines such as Benadryl, Claritin, Zyrtec, and Alavert. Decongestants may work for you too, such as Sudafed, but they can give you a super dry throat and unfortunate sexual side effects. Or you may benefit from getting right to the source of your nasal congestion with prescription nasal sprays such as Nasonex or Flonase. Once you deal with the mucus in your nose, then you won't have to worry about dealing with that annoying post nasal drip.

Fit's Tips: If you're not into taking medications to deal with your allergies, then pick up a neti pot and do some nasal cleansing.
