How to Use This Medicine

Injectable

Your doctor will prescribe your exact dose and tell you how often it should be given. This medicine is given as a shot under your skin.
This insulin combination usually starts to work about 30 minutes after it has been injected. This insulin combination may keep working for as long as 24 hours after the injection, but it slowly works less and less. The way this insulin combination works for you might be different. You and your health caregiver must work together to know the best times for you to use your insulin.
You will be taught how to give your medicine at home. Make sure you understand all instructions before giving yourself an injection. Do not use more insulin or use it more often than your doctor tells you.
There are many different devices available for giving an insulin injection. You may be taught how to use a regular syringe, a pen with cartridges, or some other device. Each device has special instructions that you must follow. Make sure you understand all the instructions for your device before you use it.
Know what your usual kind of insulin should look like. Look at the insulin each time you are getting ready to give an injection to make sure it still looks the same. Most insulin should not be used if it has changed color or looks too cloudy or thick or has particles.
This insulin combination must be carefully shaken or the bottle rotated until the mixture is uniform. Do not use if the white material is still on the bottom of the bottle after gentle and careful mixing.

Weekend Reading: 50/50 by Dean Karnazes

You might know Dean Karnazes as the Ultra Marathon Man. In fact that is the url to his fancy website. Not only can this man run, run and run, he also writes entertaining and motivating books.

The subtitle of Karnazes' latest book, 50/50 ($25), which he co-authored with Matt Fitzgerald, explains the book: "Secrets I Learned Running 50 Marathons in 50 Days-and How You Too Can Achieve Super Endurance." I will admit, I find the thought of running a single marathon daunting, let alone 50 races back to back, in 50 different states. The endurance I gained from this book can be measured in pages per night. I flew through this book and thoroughly enjoyed it.

The tone of the book is very chatty as Karnazes explains how the idea for doing 50/50 evolved and how it came to pass. He details every race and peppers his prose with helpful running tips like what to eat, what to wear, how to catch your second wind. Mostly, he wants you to get out and go for a run. Karnazes emphasizes the importance of listening to running advice and then experimenting with it to find out what works for you and your training regimen. After burning the midnight oil many nights reading, I always woke up inspired to lace up my sneakers and hit the road. As a runner who struggles with both speed and endurance, I appreciated his humble preface where he repeatedly states that he is just an average guy. His average-ness is our opening to create and meet our own challenges. I think both running newbies and old timers will enjoy this book. When This Medicine Should Not Be Used

You should not use this medicine if you have had an allergic reaction to norethindrone or mestranol, or if you are pregnant. You should not use this medicine if you have heart disease, angina (chest pain), a blood vessel disorder, problems with your heart valves, or uncontrolled high blood pressure. You should not use this medicine if you have diabetes, liver disease, unusual vaginal bleeding, or headaches. You should not use this medicine if you have breast cancer, liver cancer, or cancer of the uterus. You should not use this medicine if you have ever had a stroke, problems with blood clots, or jaundice caused by pregnancy or birth control pills. You should not use this medicine during pregnancy.

How to Use This Medicine

Tablet

Your doctor will tell you how much of this medicine to use and how often. Do not use more medicine or use it more often than your doctor tells you to.
This medicine comes with patient instructions. Read and follow these instructions carefully. Ask your doctor or pharmacist if you have any questions.
This medicine comes in a Dialpak® tablet dispenser set up for Sunday start. The instructions in package are revised from time to time as important new medical information becomes available. Therefore you should read the label that comes along with medicine carefully every time you get a new supply of this medicine.
The pill pack has 21 yellow "active" pills to take for 3 weeks followed by 1 week of green "reminder" pills.
Start taking this medicine on the first Sunday after your menstrual period starts. If your period starts on a Sunday, start taking this medicine on that day. Then continue taking one pill each day in the order they appear in the package.
Take one pill at the same time every day until the pack is empty. You will need to use a another method of birth control such as condoms or spermicide for the first seven days.
Start the next pack on the day after your last green "reminder" pill. Do not wait any days between packs. Birth control pills work best when there is no more than 24 hours between doses. It is very important that you take this medicine on schedule everyday.

If a dose is missed:

If you miss one active pill, take it as soon as you can. Then take your next pill at the regular time. This means you may take two pills in one day.
If you miss two active pills, take two pills as soon as you can. Then take two pills on the next day. Then go back to your regular schedule of taking one pill every day. Use another kind of birth control until you have been taking active pills for seven days in a row.
If you miss three or more active pills, do not take the pills you missed. Go back to taking one pill every day, starting with the pill for the day you remember. Throw out the rest of the pack, use another kind of birth control until you have been taking active pills for seven days in a row.
If you miss any of the 7 green "reminder" pills in 4th week, go back taking one pill every day until the pack is empty. Throw away the pills you have missed.
You could have light bleeding or spotting any time you do not take a pill on schedule. The more pills you miss, the more likely you are to have bleeding.
Take one pill daily at the same time. If you miss pills you could get pregnant.

How to Store and Dispose of This Medicine

Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light.
Ask your pharmacist, doctor, or health caregiver about the best way to dispose of any outdated medicine or medicine no longer needed.
Keep all medicine away from children and never share your medicine with anyone.

Drugs and Foods to Avoid

Ask your doctor or pharmacist before using any other medicine, including over-the-counter medicines, vitamins, and herbal products.

Make sure your doctor knows if you are using medicine for seizures such as carbamazepine (Tegretol®), felbamate, oxcarbazepine, phenylbutazone (Butazolidine®), phenytoin (Dilantin®), or topiramate (Topamax®). Tell your doctor if you are using medicine to treat infection such as ampicillin, griseofulvin, itraconazole, ketoconazole, or tetracyclines (doxycycline, minocycline, Minocin®, or Vibramycin®).
Make sure your doctor knows if you are using medicine to lower cholesterol or triglycerides such as atorvastatin, or medicines to treat HIV or AIDS (such as Agenerase®, Crixivan®, Invirase®, Norvir®, or Viracept®).
There are many medicines that interact with this medicine. Make sure your doctor knows about all other medicines you are using.

Warnings While Using This Medicine

Make sure your doctor knows if you are pregnant or breast feeding. Make sure your doctor knows if you have breast lumps or a family history of breast cancer.
Make sure your doctor knows if you have heart disease, high blood pressure, blood clotting problems, or high cholesterol or triglycerides in your blood.
Make sure your doctor knows if you have liver disease, kidney disease, diabetes, a history of migraine headaches, seizures, or depression.
This medicine can cause serious side effects such as heart attack or stroke. You are much more likely to have these side effects if you smoke cigarettes, are overweight, are over 40 years of age, or have certain health problems. Talk to your doctor about these risks.
You might have some light bleeding or spotting when you first start using this medicine. This is normal and should not last long. If you have heavy bleeding or the bleeding lasts more than seven days in a row, call your doctor.
Make sure your doctor knows if you have vomiting or diarrhea while using these pills, you might need to use another kind of birth control for a few days. Ask your doctor, nurse, or other health caregiver.
Your doctor will need to check your progress at regular visits while you are using this medicine. Be sure to keep all appointments.
Make sure any doctor or dentist who treats you knows that you are using this medicine. This medicine may affect the results of certain medical tests.
This medicine will not protect you from getting HIV, AIDS, or other sexually transmitted diseases such as chlamydia, genital herpes, genital warts, gonorrhea, hepatitis B, or syphilis. If this is a concern for you, talk with your doctor.
You may need to stop using this medicine for a few weeks before and after having surgery, or if you will be on bed-rest or otherwise inactive.
Make sure to tell your doctor about all other medical problems you have, before taking this medicine.
Call your doctor for a pregnancy test if your menstrual period does not start while taking this medicine.

Possible Side Effects While Using This Medicine

Call your doctor right away if you notice any of these side effects:

Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing.
Breast lump.
Chest pain or coughing up blood.
Dark-colored urine or pale stools.
Heavy vaginal bleeding.
Lightheadedness or fainting.
Missed or late menstrual periods.
Numbness or weakness in your arm or leg, or on one side of your body.
Pain in your lower leg (calf).
Severe nausea, vomiting, loss of appetite, or pain in your upper stomach.
Sudden or severe headache, problems with vision, speech, or walking.
Swelling in your hands, ankles, or feet.

When This Medicine Should Not Be Used

You should not use this medicine if you have had an allergic reaction to ethinyl estradiol or ethynodiol diacetate. You should not use this medicine if you are pregnant or if you have liver disease, heart disease such as myocardial infarction, a blood vessel disorder or if you have unusual vaginal bleeding that has not been checked by a doctor. Do not use if you have breast cancer, liver cancer, ovarian cancer, or cancer of the uterus. You should not use this medicine if you have ever had a stroke, heart attack, problems with blood clots, or jaundice caused by pregnancy or birth control pills.

How to Use This Medicine

Tablet

Your doctor will tell you how much of this medicine to use and how often. Do not use more medicine or use it more often than your doctor tells you to.
This medicine comes with patient instructions. Read and follow these instructions carefully. Ask your doctor or pharmacist if you have any questions.
Start taking this medicine on the first Sunday after your menstrual period. If your period starts on a Sunday, start taking this medicine on that day. Then continue taking one pill each day in the order they appear in the package.
Take your pill at the same time every day. Birth control pills work best when there is no more than 24 hours between doses. It is very important that you take this medicine on schedule every day.

If a dose is missed:

If you miss one active pill, take it as soon as you can. Then take your next pill at the regular time. This means you may take two pills in one day.
If you miss two active pills in a row, take two pills as soon as you can. Then take two pills on the next day. Then go back to your regular schedule of taking one pill every day. Use another kind of birth control until you have been taking active pills for seven days in a row.
If you miss three or more active pills, do not take the pills you missed. Go back to taking one pill every day, starting with the pill for the day you remember. For example, you may forget or miss taking your pills on Monday, Tuesday, and Wednesday. If this happens, take your Thursday pill (do not take the three you missed) and continue with your regular schedule.
You could have light bleeding or spotting any time you do not take a pill on schedule. The more pills you miss, the more likely you are to have bleeding.
If you miss any inactive pills, throw away the missed pills and go back to your regular schedule.

How to Store and Dispose of This Medicine

Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light.
Ask your pharmacist, doctor, or health caregiver about the best way to dispose of any leftover medicine after you have finished your treatment. You will also need to throw away old medicine after the expiration date has passed.
Keep all medicine away from children and never share your medicine with anyone.

Drugs and Foods to Avoid

Ask your doctor or pharmacist before using any other medicine, including over-the-counter medicines, vitamins, and herbal products.

Make sure your doctor knows if you are using rifampin (Rifadin®, Rifamate®), troglitazone, phenylbutazone (Butazolidine®), medicine for seizures such as phenytoin (Depakote®, Dilantin®). Tell your doctor if you use antibiotics such as ampicillin, griseofulvin, or tetracycline (such as doxycycline, minocycline, Minocin®, or Vibramycin®).
There are many other drugs that can interact with this medicine. Make sure your doctor knows about all other medicines you are using.

Warnings While Using This Medicine

Make sure your doctor knows if you are breast feeding or if you have recently been pregnant. Tell your doctor if you have breast lumps or a family history of breast cancer.
Make sure your doctor knows if you have blood clotting problems, heart disease, high blood pressure, or high cholesterol or triglycerides in your blood.
Make sure your doctor knows if you have diabetes, depression, kidney disease, liver disease, a history of migraine headaches, seizures, or tuberculosis (TB).
Make sure your doctor knows about all the other medical problems you have before taking this pills.
This medicine can cause serious side effects such as heart attack or stroke. You are much more likely to have these side effects if you smoke cigarettes, are overweight, are over 35 years of age, or have certain health problems.
You might have some light bleeding or spotting when you first start using this medicine. This is usually normal and should not last long. However, if you have heavy bleeding or the bleeding lasts more than seven days in a row, call your doctor.
Make sure your doctor knows if you have vomiting or diarrhea while using this pills, you may need to use another kind of birth control for a few days.
Make sure any doctor or dentist who treats you knows that you are using this medicine. You may need to stop using this medicine several days before having surgery or medical tests.
Make sure any doctor or dentist who treats you knows that you are using this medicine. This medicine may affect the results of certain medical tests.
This medicine will not protect you from getting HIV/AIDS or other sexually transmitted diseases. If this is a concern for you, talk with your doctor.
Your doctor will need to check your progress at regular visits while you are using this medicine. Be sure to keep all appointments.
Call your doctor for a pregnancy test if your menstrual period does not start while taking this pills.

Possible Side Effects While Using This Medicine

Call your doctor right away if you notice any of these side effects:

Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing.
Breast lump, breast tenderness or any swelling or discharge.
Chest pain, shortness of breath, or coughing up of blood.
Dark-colored urine or pale stools.
Heavy vaginal bleeding.
Missed or late menstrual periods.
Numbness or weakness in your arm or leg, or on one side of your body.
Severe pain in lower leg (calf).
Spotting for more than 7 consecutive days while taking the active pills.
Sudden severe headache, problems with vision, speech, or walking.
Swelling in your hands, ankles, or feet.
Yellowing of your skin or the whites of your eyes.

If you notice these less serious side effects, talk with your doctor:

Acne or mild skin rash.
Depression.
Itching or white discharge from your vagina.
Loss of hair.
Mild nausea, vomiting, loss of appetite, or pain in your stomach.
Pain or discharge from your breasts.
Rapid weight gain or loss.
Trouble sleeping.
Problems with wearing contact lenses.

How to Use This Medicine

Your doctor will tell you how much of this medicine to use and how often. Do not use more medicine or use it more often than your doctor tells you to.
Carefully follow your doctor's instructions about any special diet.
If you are using this medicine without a prescription, follow the instructions on the medicine label.
Swallow the extended-release tablet, extended-release capsule, or liquid-filled capsule whole. Do not crush, break, or chew the tablet or capsule.
The chewable tablet must be chewed completely before you swallow it.
Measure the oral liquid medicine with a marked measuring spoon, oral syringe, or medicine cup. You might need to shake the liquid just before you use it.
If you are using the sublingual liquid, measure the liquid with the dropper that comes with the bottle. Squeeze the dropper to place the liquid under your tongue. Hold the liquid under your tongue for about 30 seconds, then swallow it.

If a dose is missed:

Missing a dose of a vitamin supplement is usually not a cause for concern.
If you miss a dose or forget to use your medicine, use it as soon as you can. If it is almost time for your next dose, wait until then to use the medicine and skip the missed dose. Do not use extra medicine to make up for a missed dose.

When This Medicine Should Not Be Used

You should not use this medicine if you have had an allergic reaction to salmeterol, fluticasone, or milk proteins. Do not use this medicine to treat an asthma attack or a COPD flare-up.

How to Use This Medicine

Disk, Powder Under Pressure, Liquid Under Pressure

Your doctor will tell you how much of this medicine to use and how often. Your dose may need to be changed several times in order to find out what works best for you. Do not use more medicine or use it more often than your doctor tells you to. Using too much medicine can cause serious side effects.
This medicine should come with a Medication Guide. Read and follow these instructions carefully. Ask your doctor or pharmacist if you have any questions. Ask your pharmacist for the Medication Guide if you do not have one. Your doctor might ask you to sign some forms to show that you understand this information.
Advair Diskus® and Advair® HFA are used with other medicines to treat your breathing problems. Use all other medicines your doctor has prescribed as part of your combination treatment. Keep using these medicines for the full treatment time, even if you feel better after the first few doses.
Advair Diskus® is a powder that is used with its own special inhaler device. Individually measured doses of the powder are held in a foil blister package that is placed into the Diskus®. The Diskus® pierces (pokes a hole) in each blister one at a time to load the correct dose into the chamber when you are ready to use the medicine. Do not use the Diskus® with a spacer.
If you use the Advair Diskus® form: Keep the Diskus® inhaler closed when you are not using it. Keep the inhaler dry at all times. Do not blow into it.
If you use the Advair Diskus® form: Before inhaling your dose, breathe out fully, trying to get as much air out of the lungs as possible. Hold the Diskus® level and away from your mouth.
If you use the Advair Diskus® form: Open your mouth and breathe in quickly and deeply through the Diskus®. Do not breathe in through your nose. Remove the Diskus® from your mouth. Hold your breath for about 10 seconds or as long as possible, then breathe out slowly.
Advair® HFA is a liquid. You will use this medicine with a device called a metered-dose inhaler. The inhaler fits on the medicine canister and turns the medicine into a fine spray that you breathe in through your mouth and to your lungs. You may be told to use a spacer, which is a tube that is placed between the inhaler and your mouth. Your caregiver will show you how to use your inhaler and the spacer (if needed).
If you use the Advair® HFA form: Shake the inhaler well just before each use. Avoid spraying this medicine into your eyes.
If you use the Advair® HFA form: Test spray in the air before using for the first time or if the inhaler has not been used for a while.
If you use the Advair® HFA form: To inhale this medicine, breathe out fully, trying to get as much air out of the lungs as possible. Put the mouthpiece just in front of your mouth with the canister upright.
If you use the Advair® HFA form: Open your mouth and breathe in slowly and deeply (like yawning), and at the same time firmly press down on the top of the canister once.
If you use the Advair® HFA form: Hold your breath for about 5 to 10 seconds, then breathe out slowly.
If you use the Advair® HFA form: If you are supposed to use more than one puff, wait 1 to 2 minutes before inhaling the second puff. Repeat these steps for the next puff, starting with shaking the inhaler.
When you have finished all your inhalations, rinse your mouth out with water.

If a dose is missed:

If you miss a dose or forget to use your medicine, use it as soon as you can. If it is almost time for your next dose, wait until then to use the medicine and skip the missed dose. Do not use extra medicine to make up for a missed dose.

How to Store and Dispose of This Medicine

Keep the medicine in the foil pouch until you are ready to use it. Store at room temperature, away from heat and direct light. Do not freeze.
Ask your pharmacist, doctor, or health caregiver about the best way to dispose of the used medicine container and any leftover medicine. You will also need to throw away old medicine after the expiration date has passed. Throw the Advair Diskus® away 1 month after it is removed from the foil pouch or after all the blisters have been used.
Keep all medicine away from children and never share your medicine with anyone.

Drugs and Foods to Avoid

Ask your doctor or pharmacist before using any other medicine, including over-the-counter medicines, vitamins, and herbal products.

This medicine should not be used together with similar inhaled medicines such as arformoterol (Brovana?), budesonide/formoterol (Symbicort®), formoterol (Foradil®, Perforomist?), or salmeterol (Serevent®).
Make sure your doctor knows if you have used medicine for depression (such as amitriptyline, doxepin, nortriptyline, Elavil®, Pamelor®, Sinequan®) or an MAO inhibitor (such as Eldepryl®, Marplan®, Nardil®, Parnate®) within the past 2 weeks.
Tell your doctor if you are using medicine to treat depression (such as nefazodone, Serzone®), medicine to treat an infection (such as clarithromycin, itraconazole, ketoconazole, telithromycin, Biaxin®, Ketek®, Nizoral® Sporanox®), or medicine to treat HIV or AIDS (such as atazanavir, indinavir, nelfinavir, ritonavir, saquinavir, Crixivan®, Fortovase®, Invirase®, Kaletra®, Norvir®, Reyataz®, Viracept®).
Make sure your doctor knows if you are also using any diuretics or "water pills" (such as furosemide, hydrochlorothiazide [HCTZ], torsemide, Demadex®, Lasix®) or blood pressure medicines (such as atenolol, metoprolol, propranolol, Inderal®, Tenormin®, Toprol®).

Warnings While Using This Medicine

Make sure your doctor knows if you are pregnant or breastfeeding, or if you have heart problems, high blood pressure, or seizures. Tell your doctor if you have thyroid problems, diabetes, liver problems, herpes simplex in your eye, tuberculosis, osteoporosis, or any type of infection.
This should not be the first medicine you use for asthma. It should only be used along with other asthma medicines such as corticosteroids that are taken by mouth or inhaled (cortisone, prednisone, Azmacort®, Cortone®, Deltasone®, Flovent®, Prelone®, Pediapred®, Vanceril®).
Before you start using this medicine, tell your doctor if your breathing problems have been worse recently. Tell your doctor if you have had to use more medicine to help your breathing, or if you have been to the emergency room lately for breathing problems.
This medicine will not stop an asthma attack that has already started. You should have another medicine to use in case of an acute asthma attack or for sudden COPD flare-ups.
If any of your asthma medicines do not seem to be working as well as usual, call your doctor right away. Do not change your doses or stop using your medicines without asking your doctor.
You may get infections more easily while using this medicine. Tell your doctor right away if you have been exposed to chickenpox or measles.
Patients with COPD may be more likely to have pneumonia. Call your doctor right away if you start having increased sputum production, change in sputum color, fever, chills, increased cough, or increased breathing problems.
Your doctor will need to check your progress at regular visits while you are using this medicine. Be sure to keep all appointments. You should have regular eye exams while using this medicine.

Possible Side Effects While Using This Medicine

Call your doctor right away if you notice any of these side effects:

Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing.
Chest pain.
Difficulty swallowing.
Eye pain or trouble seeing.
Fast, pounding, or irregular heartbeat.
Fever, chills, cough, runny or stuffy nose, sore throat, and body aches.
Tremors, nervousness, or shaking.
Worsening of breathing problems.

If you notice these less serious side effects, talk with your doctor:

Bone or muscle pain.
Dry mouth or white patches inside mouth.
Headache.
Hoarseness or voice changes.
Menstrual problems.
Nausea, vomiting, or upset stomach.
Pain or weakness in your muscles or joints.
